Addition Clinical Encounters

To maintain fair market value while ensuring completely open, flexible access to your provider:

• Extended Encounter Rate: $70 per additional clinical encounter.

Applies universally across all three package structures if a patient exceeds their allotted 6-month visit limits. Covers both comprehensive virtual telehealth consultations and in-person medical evaluations.

In-Office Diagnostic Testing Menu

Rapid, medical-grade diagnostic testing performed instantly right in our office. These flat-rate add-on prices drastically undercut traditional South Florida urgent care fees to ensure seamless, transparent care.

• 12-Lead ECG: $40 (Includes full tracing, physical printout, and formal healthcare provider interpretation)

• Respiratory Combo Swab: $30 (Advanced Multiplex rapid nasal swab testing for COVID-19 and Influenza A & B simultaneously)

All medical services are available by appointment only. To fit your lifestyle and clinical needs, your scheduled care may include comprehensive virtual telehealth consults or in-person clinical encounters at our office. Simply text or call the clinic and you will be swiftly accommodated.

Medication & Billing Policies

• Routine Medications: Standard, non-narcotic prescriptions are included in your package encounters and e-prescribed directly to your local, in-network commercial pharmacy utilizing your traditional health insurance plan. Narcotics / Controlled Substances requires in-person clinical encounter.

• Specialized Compounds: All specialized weight loss (GLP-1s), HRT, TRT, and longevity / anti-aging therapies are handled as a separate inventory expense. Patients pay the clinic directly for these compounds at a fixed, transparent rate competitively priced just above wholesale pharmacy invoice cost.

• Billing & Automatic Renewal: Package investments are billed securely upfront every 6 months. A valid credit card or debit card is kept safely on file. To protect clinical continuity, plans renew automatically on Day 181 unless a 30-day written cancellation notice is submitted by the member.

• Insurance Reimbursement: Our clinic is a cash-pay, out-of-network provider. However, our rendering provider is fully credentialed with the Council for Affordable Quality Healthcare (CAQH). Upon request, you will be provided a detailed medical invoice containing all legal diagnostic, CPT, and ICD-10 taxonomy codes. You may submit this document directly to your commercial health insurance provider to have paid fees applied toward your annual out-of-network deductible or partial reimbursement.
